Why this matters

Your clients trust you with their goals, limitations, and quirks. That knowledge is in your head — we need it on paper so whoever picks up each client walks into their first session already knowing who they're training. Trainer assignments are still being worked out; fill in what you know for every client regardless of who ends up with them.

Answer every question. Radio/checkbox fields only take a second; the short-text boxes just need specifics. The gaps matter: if you can't answer "Did you run a day-one assessment?" the new trainer will know to start there — no judgment, just continuity.

Clients are tiered by recent activity. HIGH/MEDIUM clients (the ones you've seen lately) are the priority — plan ~10-12 min each. LOW/DORMANT clients are much faster — ~3-5 min each; most answers will be "Don't know" or a quick recall of why they stopped coming. For LOW/DORMANT clients, a 12th "Churn & re-engagement" section asks what happened and whether they're worth re-engaging.

Total estimate: ~2-2.5 hours across all 26 clients, split however works for you — one hour now, finish later. You don't have to do them in order.

If we missed any clients (off-book, cash, referred by someone, booked under another instructor), add them in the box below before you start. Your work auto-saves locally and syncs live to Alex — no need to "submit." The green synced badge at the top confirms your answers are reaching him. You can close the tab and come back; nothing will be lost.
